1. Services Provided

Dr. Angela Berry offers court-approved Alcohol and Drug Screenings/Evaluations in accordance with the Arizona Department of Health Services (ADHS) and Motor Vehicle Division (MVD) requirements under approval codes DUI0117 and DVS0113. All evaluations are performed personally by Dr. Berry.

2. Appointment Scheduling

Appointments can be booked by phone, text, or online. Clients are responsible for providing accurate information and arriving on time. Same-day and next-day appointments are available on a limited basis.

3. Payment Policy

All payments are due at the time of service by cash, cashier’s check, or money order.
Personal checks, credit cards, or digital payments are not accepted. There are no deposits required at the time of booking.

4. Cancellation and Rescheduling

If you need to cancel or reschedule, please notify us at least 24 hours in advance. Repeated no-shows or late cancellations may affect your ability to schedule future appointments.

5. Confidentiality and Compliance

All client records, documentation, and evaluations are strictly confidential and managed in compliance with ADHS regulations and Arizona state law. Reports are shared only with authorized agencies such as the court, probation officer, or MVD, as required.

6. Accuracy of Information

Clients must provide truthful and complete information during their evaluation. Providing false or misleading information may result in invalid documentation and may affect court or MVD compliance.

7. Legal and Professional Disclaimer

Dr. Angela Berry’s evaluations are behavioral health assessments designed to meet legal and administrative requirements. They do not constitute legal advice or medical diagnosis. Clients should consult their attorney or healthcare provider for additional guidance if needed.

8. Limitation of Liability

While every effort is made to ensure accuracy and timely submission of documentation, Angela Berry Alcohol and Drug Screenings/Evaluations is not liable for delays, rejections, or administrative issues beyond its control once reports are submitted to third-party agencies.
